How to request your export

  1. 1. Sign in to the client portal: your practice will have provided a portal invitation by email or SMS.
  2. 2. Open Preferences from your account menu, then find Download My Data.
  3. 3. Preview the contents: confirm what the export will contain before you submit the request.
  4. 4. Download: you'll receive a download through your browser. Save it somewhere secure: the file contains personal information.

Need clinical notes?

Clinical notes authored by your practitioner aren't part of this self-serve export. Under APP 12 you still have the right to access them, so please contact your practitioner directly. Your practitioner will assess the request under applicable access laws, including any exceptions, and respond within the applicable timeframe. Contact the practice if you need help with an access request.
